Diagnostic value of combined sIgG and sIgE of Aspergillus fumigatus in allergic broncho pulmonary aspergillosis and severe asthma with fungal sensitation

Abstract
<b>Objective:</b> Allergic broncho pulmonary aspergillosis (ABPA) and Severe asthma with fungal sensitation (SAFS) is very similar in clinical symptoms. This study aims to explore the characteristic of these patients and accurate diagnosis between them. <b>Methods:</b> The specific immunoglobulin E (sIgE) of <i>Aspergillus fumigatus</i> was detected by Phadia 1000 system and the specific immunoglobulin E (sIgG) of <i>A.fumigatus</i> was detected by ELISA kit (DRG) in 17 patients with ABPA and 14 patients with SAFS. <b>Results:</b> The levels of sIgG [2294.00 U/mL (1527.00, 14170.00) vs. 972.60 U/mL (650.90, 1792.00)], sIgE [8.77 kU/L (1.64, 16.85) vs. 1.04 kU/L (0.70, 2.05)] in ABPA patients were significantly higher than those in SAFS patients (P < 0.0 5). <i>A, fumigatus</i> sIgG was strongly correlated with <i>A.fumigatus</i> sIgE (rs = 0.797, P < 0.001) in ABPA. When combined with <i>A.fumigatus</i> sIgG (> 1000.00 U/mL) and <i>A.fumigatus</i> sIgE (> 1.00 kU/L) for the differential diagnosis of ABPA and SAFS, the sensitivity and specificity reached 82.3% and 78.6%, respectively. There was no significant difference in blood routine, induced sputum and G test between ABPA and SAFS patients. <b>Conclusion:</b> ABPA patients have higher levels of sIgG and sIgE of <i>A.fumigatus</i> than SAFS patients. Combination of sIgG and sIgE of <i>A.fumigatus</i> to diagnosis ABPA and SAFS patients is important.
